All students enrolled in the Academy of Angel classes need a current physical and immunization record on file. Please be sure you are in compliance by their first day of school.

Update in Final Forms

K and 7th grade: Students entering kindergarten and 7th grade are required to receive updated immunizations. Please be sure to send a copy of the updated immunization record to school. Students not in compliance will not be able to attend school after day 14.

1. Students who have an asthma inhaler or epinephrine injector will need a release form signed by the physician.

2. Students who will need to take prescribed medications during school hours will need a Physician Request form signed by the physician.

3. Students with allergies to food and/or anaphylaxis should complete an Allergy Care Plan.

4. Medical supplies can be brought to school office during August hours or at our Back-to-School Night.
